Are there any maintenance or grounds issues to be reported to Council? Refer to the Sport and Recreation Facility Guide - Appendix 1 & 2.
Use the Sports Facility Maintenance Request form to report these issues before continuing with this form.

As per the Facility Guide, Clubs are responsible for providing bins to manage waste generated as a result of their facility use. Council will supply at least one sanitary bin in public toilets at facilities. Additional sanitary bins are the users responsibility to arrange.
