Form process

Council systems will be updated after submitting this form and a Council officer will contact either party if there are any outstanding questions. This usually occurs within three working days.

Both parties will receive confirmation of the success of this application.

The dog/s must wear their registration tag at all times if it is at a place other than the address stated in this form.
